Recently, I spoke to a senior emergency planner who has worked for years in the transportation and nuclear industries. From memory he recited to me a catalogue of omissions, oversights and failures in managing the risks. In the UK this is the Civil Contingencies Act of November 2004. These are rather ambiguous bodies.
